Interactions

A. Top Drug-Drug Interactions:

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).

2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.

3.Other antihypertensive drugs (e.g., atenolol, propranolol, enalapril, lisinopril): Cialis can enhance the blood-pressure-lowering effects of these medications. While this interaction is usually less severe than with nitrates or alpha-blockers, it can still cause dizziness or fainting in sensitive individuals. Close monitoring of blood pressure is recommended.

Other Drug-Drug interactions:

  • Ketoconazole, itraconazole (used to manage fungal infection)
  • Protease inhibitors (used to manage HIV infection) Ex. ritonavir, lopinavir
  • Phenobarbital, phenytoin, carbamazepine (medicines used for fits)
  • Rifampicin, erythromycin, clarithromycin (medicines used for bacterial infections)
  • Other medicines used to improve blood flow to the penis (Ex. sildenafil)

B. Drug-Food Interactions:

Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:

  1. Grapefruit and Grapefruit Juice: Grapefruit can increase the levels of tadalafil in your blood by interfering with its metabolism. This may amplify side effects, such as headaches, dizziness, or low blood pressure. Avoid consuming grapefruit or grapefruit juice while taking Cialis.
  2. Alcohol: Drinking alcohol with Cialis can enhance the blood pressure-lowering effects of the medication, leading to symptoms like dizziness, lightheadedness, or fainting. Limiting alcohol intake is recommended while using Cialis.
  3. High-Fat Meals: While Cialis can be taken with or without food, consuming it with a high-fat meal may delay its onset of action. Opt for lighter meals if rapid effectiveness is needed.
  4. Caffeine: Though not a direct interaction, combining Cialis with high caffeine intake might worsen side effects like headaches or increase heart rate in sensitive individuals.

Cautions

Heart Attack Drug:

While Cialis is generally safe and effective when used under medical supervision, it is important to consider the potential risks and side effects. While the medication can cause a drop in blood pressure when taken with a heart- health drug (such as a diuretic), always consult your healthcare provider to discuss the risks and discuss the benefits of combining it with cardiovascular health medications.

Other Drug-Food Interactions:

Cialis can interact with other medications, especially with alpha-blockers, bagged Sildenafil, or othernitrates. Medicines like isosorbide dinitrate or nitrite may worsens your blood pressure-lowering effects when taken with Cialis. Before using Cialis with any other heart- health medication, it's important to consult your healthcare provider.

A small percentage of Canadians have been denied access to and use of erectile dysfunction drugs.

The Food and Drug Administration (FDA) in the US and Canada has found that a small group of Canadians are being denied access to erectile dysfunction drugs, despite the fact that the FDA has granted a conditional approval from a panel of regulatory authorities to allow the FDA to issue a black box warning to erectile dysfunction drugs on the basis of their safety in relation to human use.

“We are concerned that the safety of erectile dysfunction drugs on the market, especially the oral medication Viagra, is not as strong as what is being proposed. In addition, the FDA is considering additional data to inform the regulatory agency,” said Dr. Brian D. McKinnell, FDA’s interim director of regulatory affairs.

“We are concerned that the FDA in the US is not considering the safety of any of the erectile dysfunction drugs that have been approved by the agency. We are concerned that the safety of the drugs that have been approved by the FDA may be as bad as the products that are being approved by the agency.”

The FDA has not approved any erectile dysfunction drug, but its black box warning does appear to be in compliance with the safety standards of the drug market.

The FDA has also received approval from the European Medicines Agency (EMA) for two new medications to treat erectile dysfunction (ED) with an effect that is similar to what is being proposed, according to a press release from the agency.

The drugs include sildenafil (Viagra) and tadalafil (Cialis).

The agency said it will work with the EMA and the European Medicines Agency to review the drugs to determine the safety of them.

“If we can identify the safety of the drugs we will work with the EMA and the European Medicines Agency,” said Dr. Paul P. Oleson, FDA’s interim director of regulatory affairs. “We will work with the EMA and the European Medicines Agency to evaluate the safety of the drugs we are currently marketing.”

There is also a “black box warning” for the drugs. The FDA has received an additional black box warning for the following ED medications:

  • sildenafil (Viagra)
  • tadalafil (Cialis)
  • avanafil (Avanafil)
  • fosamprenavir (Cymbalta)
  • revatio (Sulindac)
  • trimethoprim/sulfamethoxazole (Bactrim)
  • amikacin (amikacin)

“The agency will evaluate whether we can identify the safety of these drugs and whether we can identify the safety of our drugs,” said Dr. Paul Oleson.

The FDA is not aware of any of the black box warning drugs currently being marketed in the US and Canada. If the FDA finds that the FDA is not making the safety recommendations that are currently being made by the FDA, the agency may issue a conditional approval, but there are no other drugs that are in compliance with the FDA’s black box warning.
